Mediclassic is a limited
health insurance plan that offers affordable and superior care
through its provider network.
We have 100 primary care
physicians and 200 specialists, over 19 Centers, conveniently
located, that are open daily and weekends for your convenience.
What does it means
"limited health insurance"?
It means that this insurance
doesn't cover hospitalization expenses. But , Mediclassic works with
the BCBS of SC Mini-Medical so they can provide you with
hospitalization and emergencies.

Can I change my Primary
Care Physician?
Yes, by
completing a Membership Status Change Form, that is available at the
Centers or we could send you a form via mail.
Where do I go or call if I
need attention or care after regular office hours?
If you are
sick or injured after regular office hours, please call Your
Primary Care Physician or Clinic . (You can find the
clinic's phone number on your membership card.)
If it is an urgent or
emergency situation go to the nearest hospital emergency room or
call 911.
When does my coverage or
membership starts?
The
coverage will become effective on the first (1st) day of the month
following SFPHC's receipt and acceptance of the Member's fully
completed application , subject to the Member's satisfaction of all
eligibility requirements and the payment of applicable Premium. The
Member's Coverage Effective Date is set forth on the Plan
Information Page.
How can I terminate my
plan or coverage?
By signing
and submitting a Membership Status Change form. This termination
will be effective on the first (1st) day of the month following our
receipt of such Membership status change form provided such form is
received by SFPHC at least five (5) business days before the end of
the month preceding the month in which termination of coverage is to
be effective.
